Complete the intake form and donation to participate in Bella Vida Chapel ayahuasca or psilocybin sacrament ceremonies in Melbourne, Florida.
We do not charge for the sacraments. The donation covers the facilities and ceremony expenses. The donation link will be sent after intake form is complete.
Do not make travel arrangements until you have been medically cleared and accepted for the requested ceremony.

We do not charge for the sacraments. The donation covers the facilities and ceremony expenses.
If you choose to have someone pick you up, you will be free to leave Saturday night around 10:00 pm.
If you need overnight accommodations, we partner with a nearby oceanfront hotel where rooms are typically available for around $200 per night. Transportation to the hotel can be provided after the ceremony.
